There no subject, so the context can change the meaning, but it means "don't know what I/he/she/it is doing". There's no subject, so don't know what/who it is talking about. It is slightly more informal/impolite (along the lines of "what the heck is he doing"  . But, it could be "I don't know what the heck he is doing" "He doesn't know what the heck is doing" "I don't know what the heck I'm doing" depending on the context. Again, no subject, so can't be sure without the context ^_^ Hope that helps!~

He posted this before that thread, I just got back to the forums and saw it browsing through the last page or so.
Anyway, as another note: that phrase is fine online, and would normally be used with friends, and when joking around, having fun. More normally you would just use 不知道做什么, and put the subjects in as appropriate.
